Please Bring the Following to the Meeting

 
1. Registration forms for coaches and players
 
2. Date of birth verification for each player (accepted club level roster, town roster registrar stamped, license)
 
3. Roster per team
 
4. Payment in FULL (with 1 check from the head coach or team parent)
 

All forms & fees must be submitted before games are played.

All players and coaches will be registered with the U.S. Futsal Federation (USFF)

If you registered with another Futsal league you will need a copy of your registration ID#   

Roster:   If you have more than 12 players on a roster only 12 players are allowed to any one game. (No freeze on the  player roster)
